{"id":367,"date":"2013-06-18T12:52:56","date_gmt":"2013-06-18T12:52:56","guid":{"rendered":"https:\/\/chiromax.ro\/?p=367"},"modified":"2013-06-18T12:52:56","modified_gmt":"2013-06-18T12:52:56","slug":"gravida-si-stomatologul","status":"publish","type":"post","link":"https:\/\/chiromax.ro\/en\/gravida-si-stomatologul\/","title":{"rendered":"Gravida si stomatologul"},"content":{"rendered":"<p style=\"text-align: justify;\">Pacienta insarcinata necesita din partea medicului dentist o atentie speciala pentru ca, desi este considerata ca fiind &#8222;sanatoasa&#8221;, trebuie ferita de stres si anxietate, toate acestea deoarece organismul ei sufera o serie de modificari fiziologice si emotionale.<\/p>\n<p style=\"text-align: justify;\">Printre aceste modificari putem enumera:<!--more--><\/p>\n<ul>\n<li>cresterea volumului sanguin cu 50%, tahicardie si murmur cardiac fiziologic;<\/li>\n<li>sindrom hipertensiv la schimbari bruste de pozitie in stadii avansate ale sarcinii;<\/li>\n<li>anemie, modificari ale factorilor de coagulare;<\/li>\n<li>modificarea tesutului conjunctiv din vasele de sange;<\/li>\n<li>modificari hormonale.<\/li>\n<\/ul>\n<p style=\"text-align: justify;\">Modificarile metabolice ce apar in organismul femeii gravide pot favoriza aparitia unor complicatii orale cum ar fi:<\/p>\n<ul>\n<li>sangerarile gingivale cauzate de factori iritanti locali si care se pot combate printr-o conduita corecta de igiena orala;<\/li>\n<li>foarte rar apare tumora de sarcina &#8222;pregnancy tumor&#8221;, gingiile cresc in volum acoperind dintii, sangereaza foarte usor la atingere, sunt dureroase numai in cazul unor suprainfectari. Tumora nu are caracter malign, fiind din punct de vedere bistopatologic un angiogranulom;<\/li>\n<li>evolutia cariilor dentare se poate accentua din cauza modificarilor in alimentatie, dar nicidecum din cauza ca se pierde calciul din dintii mamei. Suplimentul de calciu pentru fat poate fi asigurat de o alimentatie bogata in calciu.<\/li>\n<\/ul>\n<p style=\"text-align: justify;\">Pentru a preintampina toate aceste posibile complicatii, recomandarea medicului dentist cuprinde:<\/p>\n<ul>\n<li>curatarea riguroasa a dintilor dupa masa pentru a reduce placa bacteriana dentara;<\/li>\n<li>utilizarea solutiilor bucale bogate in fluor, sub forma de clatiri bucale dupa periaj;<\/li>\n<li>aport alimentar bogat in vitamine (A, C, D), calciu, fosfor si cat mai putin zahar si hidrati de carbon, acestea din urma contribuind la dezvoltarea placii bacteriene dentare;<\/li>\n<li>control periodic la medicul dentist.<\/li>\n<\/ul>\n<p style=\"text-align: justify;\">In cazul necesitatii unor tratamente stomatologice curente, perioada cea mai sigura este cel de-al doilea trimestru de sarcina.<\/p>\n<p style=\"text-align: justify;\">Tratamentele protetice sau parodontale se vor temporiza pana dupa nastere, rezolvandu-se doar cele ce constituie o urgenta. Foarte des pacientele gravide se tem de administrarea substantelor anestezice. Ele sunt periculoase in primul trimestru de sarcina, alaturi de marea majoritate a medicamentelor. In aceasta perioada au loc procesele de organogeneza fetala (se formeaza toate organele) si fatul trebuie ferit de riscul mutagen al substantelor medicamentoase. Incepand cu trimestrul al doilea de sarcina se pot folosi lidocaina, prilocaina, bupivacaina.<\/p>\n<p style=\"text-align: justify;\">Important este de precizat ca trebuie evitate in perioada de sarcina: radiografia dentara, mai ales in primul trimestru de sarcina; administrarea de tetraciclina, deoarece poate afecta structura dentara a copilului.<\/p>\n<p style=\"text-align: justify;\">In trimestrul I nu este recomandat tratamentul stomatologic (exceptie fac igienizarea profesionala si controlul durerii), din cauza riscului de avort. Trimestrul II este cel mai bun pentru efectuarea tratamentelor de rutina fara ca sanatatea mamei sau a fatului sa fie in pericol, in timp ce in ultimul trimestru este bine ca tratamentul stomatologic sa fie evitat (risc de nastere prematura).<\/p>\n<p style=\"text-align: justify;\">Pentru a contribui la o buna dezvoltare a dintilor copilului se recomanda administrarea de fluorura de natriu incepand cu luna a patra la femeia gravida (2 mg. pe zi). De acest aport suplimentar de fluor vor beneficia dintii de lapte ai copilului care incep sa se formeze inca din saptamana a opta.
